The e-visa has no paper form, the visa on arrival uses form NA1, and some airports now want an online pre-arrival form. Here is every form a visitor to Vietnam might meet, where to get the official version and how to fill it in.
People search for a "Vietnam visa form" to download and print. For most travellers there isn't one any more: the e-visa is applied for entirely online. Paper forms are still used for the visa on arrival and at embassies. Here is which form you need for each route, and where the official one is.
The form you need, by route
| How you get your visa | Form | Where you get it |
|---|---|---|
| E-visa | The online application | evisa.gov.vn, or our application form |
| Visa on arrival | Form NA1, visa application form | At the airport visa counter |
| Embassy or consulate visa | Form NA1, or the mission's own version | From the embassy or consulate |
| Arriving at an airport that asks for it | Pre-arrival declaration | Online, before you fly |
We don't host copies of government forms. Old copies circulate online for years after a form changes, so always use the version from the official source.
The e-visa application
There is nothing to print. The form is completed online and includes:
- your personal details, exactly as in the passport's machine-readable lines - surname, middle and given names, in capitals;
- passport details: number, issue and expiry dates;
- your trip: purpose, intended dates, the border gates you enter and leave by, and your first address in Vietnam;
- your travel history to Vietnam in the past year;
- a portrait photo and a scan of the passport photo page, uploaded as files.
The only official addresses are evisa.gov.vn and thithucdientu.gov.vn. If you apply with us, our form asks the same questions in plain English, reads your passport scan for you, and a person checks everything before it is lodged. Start at the Vietnam e-visa page.
Form NA1: visa on arrival and embassy visas
Form NA1 is the visa application form for foreigners - a one-page form with a space for a 4x6 cm photo. You complete it:
- at the visa-on-arrival counter, together with your approval letter, passport, photos and the stamping fee in cash; or
- at a Vietnamese embassy or consulate, when you collect a visa approved through a sponsor - see Vietnam embassy visa.
How to fill it in without being sent back to the queue:
- Use block capitals and copy your name exactly as it appears in the passport.
- Give the same dates and address as your approval letter or sponsor's request.
- Name your sponsor - the agency, company or relative in Vietnam - as shown on the approval.
- Glue or staple one photo in the box and keep a second photo ready.
- Sign and date it. An unsigned form is the most common reason for being sent back.
Counters usually have blank forms. Filling one in on the plane, if your agency gave you a copy, saves time on landing.
The pre-arrival declaration
Vietnam's Immigration Department asks travellers to complete an online pre-arrival form before landing, and at Ho Chi Minh City airport it has been required since 15 April 2026. It is free, takes a few minutes and is done on the department's official site, linked from evisa.gov.vn. See the Vietnam pre-arrival form.
Forms you don't fill in yourself
- Temporary residence declaration. Every foreigner staying in Vietnam is registered with the local police. Hotels and guesthouses do this for you when you check in; if you stay with friends or in a private rental, the host does it.
- Customs declaration. Only needed if you carry goods, foreign currency or gold above the declaration limits.
- Sponsor requests. For visas that need a sponsor, the company, school or relative in Vietnam files the request with the Immigration Department - not the traveller.
Frequently asked questions
Where can I download the Vietnam visa application form?

Do I need form NA1 for an e-visa?
No. Form NA1 is only for visas issued on arrival or at an embassy.
Can I fill in the visa on arrival form in advance?
Yes, if you have a copy. Use block capitals and match the details on your approval letter exactly.
What photo size does the Vietnam visa form need?
4x6 cm, recent, facing the camera, on a white background, with no hat and no glasses - the same rule as the e-visa portrait. See Vietnam e-visa photo requirements.
Is there a form to fill in on the plane to Vietnam?
Vietnam's arrival declaration is the online pre-arrival form. Complete it before you fly: it is compulsory at Ho Chi Minh City airport and encouraged at the others.
